"My mom's care has been provided by nurses and aides that are kind, caring, and professional. They are like part of the family. We don't know how we would have kept mom at home if it wasn't for your help."There's no doubt that maintaining your independence as you grow older, or as you cope with an illness, can enhance your quality of life. If you or a loved one is in need of skilled or long-term care, we can help. As a program of Steuben County Public Health & Nursing Services, we can help you with a plan of care that can keep you or your loved one at home in reassuring and comfortable surroundings. Working in conjunction with the Steuben County Department of Social Services and other community agencies, we are able to tailor a care program for your loved one's individual needs. We also work with area hospitals to ensure that discharged patients receive continuity of care. Is my loved one eligible? If your loved one is a resident of Steuben County and receives Medicaid, he or she may be eligible for Long-term Care Services. In addition, the patient must qualify for nursing home level care and: ·
have a physician approved plan of care.
It is also important that the patient have someone who is willing to assume responsibility to serve as his or her caretaker in the event our aides cannot get to a home due to weather conditions or other emergencies. How do we apply for Long-term Care? The first step is to ask your physician, medical practitioner, hospital, or community agency for a referral. You can also call us at (607) 664-2438 to find out more information. Is there a cost? Long-term care costs are covered by Medicaid. Ability to pay is not a requirement for services. How do I know what's the best plan of care for my loved one? Our nursing staff will work with you to determine your needs. A plan of care will then be developed to keep you or your loved one comfortable in the safety and independence of their own home setting. Who will take care of my loved one? A friendly, professional member of our staff will come to your home to help you meet your loved one's day-to-day needs. Our nurse will also meet with you every four months, or more often if needed, to make sure you are continually satisfied with your plan of care. What services do you offer?
